Eligibility

Eligibility for a Pennsylvania handicap parking placard is determined by the Pennsylvania Department of Transportation (PennDOT) and requires certification from a licensed physician.

Qualifying Conditions


  • Inability to walk 200 feet without stopping to rest.
  • Inability to walk without the use of, or assistance from, a brace, cane, crutch, another person, prosthetic device, wheelchair, or other assistive device.
  • Lung disease to such an extent that the person's forced (respiratory) expiratory volume for one second (FEV1) is less than one liter or the arterial oxygen tension (pO2) is less than 60 mm/Hg on room air at rest.
  • A cardiac condition to the extent that the person's functional limitations are classified in severity as Class III or Class IV according to standards established by the American Heart Association.
  • A severely limited ability to walk due to an arthritic, neurological, or orthopedic condition.
  • A specific statutory blindness classification.
  • Amputation or a permanent loss of use of a hand or foot.
  • A mental condition found to be disabling by a physician.

How to Apply

Pennsylvania offers both temporary and permanent handicap parking placards. Temporary placards are valid for a specified period, as certified by your physician, while permanent placards are valid for five years and require renewal.

Application Process


  • Complete the Applicant section of the MV-145A Application form.
  • Have your licensed physician complete and sign the Physician's Certification section of the form, detailing your qualifying disability.
  • Get the form notized at our office.
  • Mail the completed application to the address specified on the form. Be sure to include any required fees (if applicable).

Placards Renewals


PennDOT will typically send a renewal application before your permanent placard expires. Follow the instructions on the renewal form to maintain your parking privileges.
